Incident Form

Sometimes accidents happen. If an unplanned event exposes a worker or a client to risk of injury, ill health or other loss, please report the situation to the Support Network. This is if the event occurred between a worker and client who used the Support Network to connect with each other.

Once we have received your incident report, our team will review it within 2 working days (during that time, we may get in touch to ask you some additional questions). If the client comes from a registered Aged Care or Disability provider, we will forward the incident report to that provider for follow up.

If you are a support worker, you must include the incident on your timesheet.
