Review options for practical support in terms of fit, capacity and participant choice.
A client might need in-home or community services, but feel overwhelmed by provider lists, unfamiliar service models and pressure to decide quickly.
Matching is about more than availability. The acceptance and maintenance of support is influenced by communication, culture, interests, routine, worker competencies and the person's prior experiences.
Social workers can help clarify the request and compare the safeguards or support, while ensuring the decision ultimately rests with the person.
Identify the essential skills, timing, communication and access needed, separately from what is not preferred.
Discuss capacity, worker match, cost, agreements, cancellations, supervision and escalation for each option.
If needed, use an introduction, trial or review point so the person can judge the lived fit.
A useful change is specific, so it can be practised, and small so it can be reviewed. Decide who will take the first step, record the decision and determine when the person will be asked if the step helped. If the method requires multiple services, check each service individually, rather than assume they are ready to operate because they agreed at a meeting. Keep what works, modify what doesn't and eliminate the short-term solutions that are no longer meeting the end goal.
What would “define the non-negotiables” look like from the person's point of view?
What part of the arrangement is confirmed and what part is still an assumption or request?
What will the person and team notice that will give them evidence that this approach is helping?
A referral is not an endorsement or guarantee. State what has been checked, manage conflicts of interest and don't steer the person toward one option without letting them consider all of them.
